Explore the options available to older adults and their families. Our outreach and assistance program is designed to help you navigate through all the older adult services in Kent County. Trained social workers can help you identify the services that best meet your or your loved ones needs, including assistance with the following:a
- Medicare & Medicaid
- Housing, Utilities & Home Repair
- Meals & Food Pantries
- Legal Assistance
Prescriptions
- Personal Care Services
- Transportation Coordination
- Social and Recreational Activities
We also provide assessments, education, referrals, therapeutic conversation, coaching and coordination of services. And, we’re flexible. We can provide services in our office, your home or care facility. If you are age 60 or older – or calling on behalf of someone who is – and live in Kent County, give us a call. If we do not provide a service you or your loved one needs, we can help you find it!
