For more than 30 years, Gerontology Network has been focused on the health and wellbeing of older adults in West Michigan. We pride ourselves in providing quality services, while meeting the ever-changing needs of adults as they age at home. From personal care assistance to case management, we have a variety of services that help people maintain their independence and the lifestyle they hold dear in the home they love.

Continuing Care Services
Counseling and Case Management - Develop a personal plan to enhance mental and physical health. Certified staff provide professional counseling services, including emotional support for individuals experience depression, anxiety, life transition issues, substance use, caregiver stress, or any combination of these. Our specialists work closely with clients to build a personalized treatment plan that centers around the needs of the older adult, their caregivers or families. We offer psychiatric and nursing support, in addition to a number of other services to help maintain independence and increase social contact of older adults, support care providers, strengthen families and help individuals to live full and complete lives.

Outpatient Therapy
Substance Abuse/Co-occurring Dependencies - Gerontology Network offers substance abuse and mental health follow-up care, relapse prevention and educational programs for individuals - and their families - struggling with substance abuse and co-occurring dependencies. We typically serve older adults - age 55 and older - and their families, as we help them confront issues related to substance use and dependencies. Adults younger than 55 may also be considered for our programs and services. All requests are confidential and are in strict accordance with our privacy policy.

Outreach and Assistance( Learn More > )
Explore the options available to older adults and their families. Our Outreach & Assistance program is designed to help you navigate through all the older adult services in Kent County. We can help you identify the services that best meet your - or your loved ones - needs, including assistance with housing, utilities, meals, prescriptions, social and recreational activities, and Medicare and Medicaid. We also provide assessments, education, referrals, therapeutic conversation, coaching and coordination of services. And, we're flexible. We can provide services in our office or your home.
